Legally change your child’s name

If you want to legally change your child’s name in the Northwest Territories, you must apply through Vital Statistics. This page explains how to apply for a legal change of name for a child in the Northwest Territories, including eligibility, consent requirements, and next steps.

Make sure you are eligible

You can apply for a legal change of name for your child if you:

• are at least 19 years old
• are a Canadian citizen, permanent resident or otherwise legally entitled to live in Canada
• have lived in the Northwest Territories for at least three months in a row within the last year
• have lawful custody of the child

How name changes are published

Legal name changes are published in the NWT Gazette.

The Registrar may choose not to publish your child’s change of name if:

• the change is minor
• the child has been commonly known by the new name
• publication would cause undue hardship

1. Download the Application for Change of Name, consent of parent or legal guardian and consent of child – 12 years of age or over forms.
2. Complete the appropriate sections.
3. Review the spelling of your child’s new name exactly as you want it recorded.
4. Make sure all required consents are signed, including consent from anyone with lawful custody of or access to the child, the child’s consent if they are 12 years of age or older, and a spouse’s consent if required.
5. Email or fax a copy of your completed application to Vital Statistics if you would like it reviewed before you mail it.
6. Include payment for the required fee.
7. Mail the original application package, supporting documents and payment to Vital Statistics.
