Legally change your name (adult)

If you are an adult who wants to legally change your name in the Northwest Territories, you must apply through Vital Statistics. This page explains how to apply for a legal change of name for yourself in the Northwest Territories, who is eligible, and what happens after you submit your application.

Make sure you are eligible

You can apply if you:

• are at least 19 years old
• are a Canadian citizen, permanent resident or otherwise legally entitled to live in Canada
• have lived in the Northwest Territories for at least three months in a row within the last year

People under 19 may qualify only if they meet all eligibility requirements and are a spouse or have been a spouse.

How name changes are published

Legal name changes are published in the NWT Gazette.

The Registrar may choose not to publish your change if:

• the change is minor
• you have been commonly known by the new name
• publication would cause undue hardship

Know which form to use

Use this application only to legally change your own name.

Do not use this form if you want to:

• use your spouse’s or common‑law partner’s last name (use the spouse surname page)
• resume a previous surname after separation (use the resume your previous surname page)

1. Download the Application for Change of Name (adult).
2. Complete the appropriate sections.
3. Review the spelling or your new name exactly as you want it recorded.
4. Email or fax a copy to Vital Statistics if you would like it reviewed before you mail it.
5. Include payment for the required fee.
6. Mail the original application package, supporting documents and payment Vital Statistics.

• Change of Name certificate: $159
• Change of Name certificate (duplicate): $32
• Each additional individual included on an adult application: $64

Payment options: 

• Credit card
• Cheque or money order payable to Government of the Northwest Territories

Vital Statistics does not send a confirmation after you submit your application. They will contact you only if information is missing, unclear, or additional steps are required.

If your application meets all requirements, your legal name will be changed and recorded in the NWT registry.
