If you want to change the sex designation on your NWT birth registration, you must apply through Vital Statistics. This page explains how to apply to change the sex designation on an NWT birth registration, which form to use, and what documents you must submit.

Make sure you are eligible

You can apply only if your birth was registered in the Northwest Territories.

Choose the form that applies to you

Use the form that matches your situation:

• 19 years of Age or Older if you are 19 years of age or older.
• 16-18 Years of Age, Independent Minor if you are 16 to 18 years old, live independently from your parents and have no legal guardians.
• Under 19 Years of Age if you are under 19 years old and a parent or legal guardian is applying on your behalf.

What changing your sex designation affects

Changing your sex designation updates the information on your NWT birth registration and birth certificate.

This process:

• does not legally change your name
• does not automatically update records held by other organizations

1. Download the Change of Sex Designation form that applies to your situation.
2. Complete all required sections of the form.
3. Sign the form before an authorized official:
   • If you sign the form in the Northwest Territories, it can be signed before a Commissioner of Oaths, Justic of the Peace or Notary Public.
   • If you sign the form outside the Northwest Territories, it must be signed before a Notary Public.
4. Include all required supporting documents for your application type. This may include written statements, professional declarations, consent forms or waiver forms.
5. Return all previously issued NWT birth certificates.
6. Complete the Application for Certificate of Birth if you want a new birth certificate showing updated sex designation.
7. Include payment for the required fees.
8. Mail the original completed forms, supporting documents and payment to the Registrar General of Vital Statistics.

• Change of sex designation amendment: $39
• New NWT birth certificate: $26

• Cheque or money order payable to Government of the Northwest Territories

Vital Statistics does not send a confirmation after you submit your application. They will contact you only if information is missing, unclear, or additional documentation is required.

If your application meets all requirements, your birth registration will be updated and a new birth certificate will be issued.

Other organizations, such as those that issue passports or driver’s licences, have their own requirements. They may not accept all sex designations.
