Replace a Change of Name certificate

If you have legally changed your name in the Northwest Territories and no longer have your Change of Name certificate, you can request a replacement. This page explains who can apply, what information you need, and how to submit your request. 

Make sure this is the right page

Use this page if:

• your legal change of name has already been registered, and
• you need a replacement or additional copy of your Change of Name certificate.

Do not use this page if you want to legally change your name.

Gather the documents and information you need

Prepare the following before you apply:

• Your current name and former name
• Your date of birth
• The date your name change was registered (or an approximate date)

1. Download the Application for a Duplicate Change of Name Certificate.
2. Complete all sections of the form clearly and accurately.
3. Include details of the name change, including your former and current names and the registration date.
4. Include payment for the required fee.
5. Submit your final application and fee using the method you choose.

Vital Statistics does not send a confirmation after you submit your application. They will contact you only if information is missing, unclear, or required consents are incomplete.

If your application meets all requirements, they will issue a duplicate Change of Name certificate.
