Update your NWT health care information when your personal details change. This page explains when you need to update your information and how to submit the Notification of Change Form.
Know when to update your information
Update your information if you change your:
- Name
- Address or phone number
- Gender
- Citizenship or immigration status
- Indigenous status
Not updating your information could interrupt your health care coverage.
Do not use this page if you need to:
- Replace a lost or damaged card. Use Replace your NWT health care card.
- Apply for your first card. Use Get an NWT health care card.
- Apply for your newborn. Use Get a health care card for your newborn.
You must have a valid NWT health care card before submitting an update.
What you need to complete your update
You may need documents that show:
- Proof of your new name
- Proof of your new address or phone number
- Proof of changes to your gender, citizenship, immigration or Indigenous status
- Any additional documents listed on the Notification of Change Form
Keep copies of the form and all documents you submit.
How to update your information
- Download the Notification of Change Form.
- Gather any documents that support the change you are reporting.
- Complete every section of the form.
- Submit your form and documents by email, mail, or fax to the Health Services Administration Office.
- Keep a copy of your form and documents for your records.
