Know when to request a replacement
Request a replacement card if your current card is:
- Lost
- Stolen
- Damaged
- Unreadable
You do not need to fill out a form for a replacement card.
A replacement does not change your coverage or eligibility.

What you need before you contact the Health Services Administration Office
You only need enough information to verify your identity, such as:
- Your full name
- Date of birth
- Current contact information
- Any other details requested by Health Services Administration during verification
You do not need to prepare a form or submit documents unless asked.
How to request a replacement card
- Call or email the Health Services Administration Office.
- Provide your name, date of birth, and any other details requested to verify your identity.
- Confirm your current mailing address.
- The Health Services Administration Office will mail your replacement card.
There is no fee to replace your card.
