Make sure you can order a certificate
You can order a death certificate if you are any of the following:
- A member of the immediate family or next of kin
- A person who needs the certificate for court, estate settlement, or disinterment
- An officer of the Crown for official purposes
- A government department approved by the Director of Vital Statistics for legitimate purposes

A certificate can only be issued if the death occurred in the Northwest Territories and has already been registered.
What the death certificate includes
A death certificate contains key details that verify the identity of the deceased and confirm the official record of death. It includes:
- Full legal name of the deceased
- Date of death
- Place of death
- Gender
- Age
- Mariage status
- Name of spouse
- Registration details

- Complete the appropriate sections.
- Submit your application and fee using the method you choose.
- Keep a copy of your application for your records.
